What Does a Community Health and Wellbeing Worker Do?

Our Community Health and Wellbeing Workers are here to support you and your household to live healthier, happier, and more connected lives. We work closely with your GP practise to make sure you receive the right help at the right time

How the Community Health and Wellbeing Programme Works

  • Each household is matched with a named worker who becomes your main point of contact.

  • Support is available to every person living in the household, regardless of age or circumstance.

  • Your GP practice allocates your household to us, ensuring we are linked into your existing healthcare team.

  • Your named worker stays with you for as long as you need, as long as you remain registered with the GP practice.


How We Can Support You

  • We tailor our support to you. Your health and wellbeing plan is personalised to your needs, goals, and preferences.

  • We take the time to listen. You can talk to us about your social, health, or wellbeing concerns in a non-judgmental, confidential space.

  • We empower you to take control. We provide tools, information, and resources to help you make informed decisions about your wellbeing.

  • We connect you to the right support. This may include referrals to organisations, signposting to services, or linking you with people who can help.

  • We keep you informed about health opportunities, such as available screenings, vaccinations, and local health initiatives.

  • We help you get involved in your community. We can introduce you to local activities, groups, and events—or even advise you on setting up your own if you’re interested.
